Registration for School

To register you will need the following 6 items as well as a filled out registration packet which is included in four separate parts at the bottom of this webpage

1) Birth Certificate    
2) Doctors Physical Report of Health Examination for School Entry Informe del examen de salud para el ingreso a la escuela

3) Proof of Residence (utility bill, rental agreement, or escrow papers)

   
4) Oral Health Assesment Oral Health Assessment

Evaluacion de la salud oral
5) Immunization Records    
6) Parent Photo ID    

 Packets may be picked up early from the office beginning March 1 anytime from 7:00 - 4:00
