Meeting Room Reservation Form

Submit one form per meeting. Multiple rooms can be reserved as long as dates and times coincide. If you are submitting more than 3 dates, use the comment box provided at the bottom of the form for additional dates. You may also specify date ranges in the comment box for a repeating meeting, i.e., Starting 09/01/2011, Ending 05/03/2012, every first Thursday of the month, 9:00 a.m. to 10:00 a.m.

Note:  You will receive two notifications, a web-based notice to confirm your request has been submitted and an email to confirm the room reservation(s).  Invitations can automatically be emailed to attendees.  If you would like to take advantage of this service, list full email addresses separated by commas in the comment box above.
